Private Lesson Agreement Form

Thank you so much for booking a private lesson! Please see the terms below.

  1. Cancellation Policy: We understand that unavoidable cancellations may occur. However, if cancellation happens within 48 hours before the lesson, you agree to pay the lesson fee in full by bank transfer, PayPal, PayPay, or credit card.  

  2. Payment: Acceptable payment methods include cash (please bring exact change), PayPay, bank transfer, PayPal, or credit card before or on the day of your lesson. PayPal and Credit Card payments require an additional ¥1000 processing fee.

  3. Timing: Access to the studio is granted precisely at the start time. Please do your best to arrive a few minutes early to ensure a smooth process getting into the studio. If you are running late, please let your instructor know. 

  4. Photo/Video Policy: You are welcome to set up your phone to take videos from the lesson. If you would like to share content from the course, kindly tag @chroma.tokyo. We will respect your privacy and refrain from posting any content without your permission.

  5. Responsibility for Physical Condition: You acknowledge that you are in good health and physical condition and do not have medical or other conditions that would hold you at risk during the lesson. If at any time you believe that you are not in proper physical condition, you will inform your instructor.

  6. Release of Liability and Assumption of Risk: Dancing is a physical exercise that involves the risk of injury. Although the instructor will endeavor to reduce the risk of injury through the use of proper technique, there is no guarantee that you will not be sore or experience an injury. Notwithstanding this risk, you acknowledge that you are voluntarily participating in training. You waive and release all claims, now known or hereafter known, against the instructor on account of personal injury, illness, pain, or financial loss arising out of or attributable to dance lessons.

  7. You agree to come in with an open mind, ready to learn and dance in a fun and safe space! <3
